Wellness Services

The Willow Creek San Martín Community is a full-service Memory Care facility with quality of care a top priority.

Residents can start out with basic resident services and add higher levels of assistance as they become desirable. Memory Care and Day Care services are tailored to the requirements of the individual resident.

Respite Care is also available in the Memory Care building.

Willow Creek Caregivers work closely with the residents' physicians to comply with their Plan of Care and with the families to ensure satisfaction.
