At Willow Creek Memory Care at Buffalo, we are dedicated to caring for individuals with Alzheimer’s and other dementias in an environment of dignity, security, comfort, and love. We don’t see a disease, we see a person. We understand the emotional and physical toll these conditions can have on seniors and their families, and we strive to make difficult situations manageable for all involved.
Because the effects of dementia are progressive and residents’ needs are continually changing, Willow Creek provides each resident with an individualized service plan that is derived thorough an individual assessment of needs, interests, daily routines, personal history, and preferences.
The plan anticipates and meets each person’s unique circumstances and helps residents feel safe while adding genuine meaning to their days. These plans are reviewed with you regularly by our staff to ensure that we meet the changing needs of our residents. Knowing as much as possible about each person helps staff interact with residents on a very personal level and provide care more effectively.
At Willow Creek Memory Care We Provide:
- Nurses / Medication Assistants – 24 hours a day
- Caregivers with additional certified training specifically designed for residents with dementia
- A purposefully designed environment, including interior and exterior wandering paths, and a home-like setting with familiar living spaces
- A safe and secure environment, with all exterior entrances secured and alarmed at all times to ensure the residents’ safety and well being
- A holistic approach to caring for the residents, as specially trained staff are responsible for caring for all aspects of residents’ daily needs
- A continuation of residents’ lifelong interests which are incorporated into their daily activities
- Well-balanced nutritious meals and snacks, including specialized diets if necessary, designed by a Registered Dietitian
- Scheduled transportation to medical appointments and social outings
- Specially designed recreational, leisure, and life skills programs planned by a full-time Enrichment Coordinator
- Monthly support groups for family members
