Respite Care

You will receive a packet with simple paperwork to fill out such as:
- Deposit form and payment agreement
- Admission agreement
- General information about your loved one, including insurance information, social security card copy and photo ID
- Emergency Contact information for family, doctors, dentists, etc.
- A variety of consent forms, releases, and policies to sign
Your primary care physician will complete and sign the physician's assessment and medical information form, letting us know what medications are required, and providing a prescription order. We will continue to work with the doctor through the resident's stay.
The participant must complete their 2-step Mantous TB test before acceptance. This is a mandate of Nevada state regulations.
Once all the paperwork is completed, we will schedule a meeting to finalize the paperwork and prepare a care plan.
When your loved one arrives, our caregivers and staff will do everything possible to make the stay a smooth one and help the new resident adjust.

The person who requires care can come to stay at a Willow Creek Community for a few days - up to a month or even more. They will receive all the Assisted Living or Memory Care they require, 24 hours per day. They will be eligible to participate in any activities at their location and have the opportunity to socialize. The Caregiver has peace of mind while they take care of other things and refresh themselves.
