Adult Day Care
- How do we sign up for Adult Day Care?
Let us know you are interested and we will help you through the process.
You will receive a packet with simple paperwork to fill out such as:
- Deposit form and payment agreement
- Admission agreement
- General information about your loved one, including insurance information, social security card copy and photo ID
- Emergency Contact information for family, doctors, dentists, etc.
- A variety of consent forms, releases, and policies to sign
Your primary care physician will complete and sign the physician's assessment and medical information form, letting us know what medications are required, and providing a prescription order. We will continue to work with the doctor through the resident's stay.
The participant must complete their 2-step Mantous TB test before acceptance. This is a mandate of Nevada state regulations.
Once all the paperwork is completed, we will schedule a meeting to finalize the paperwork and prepare a care plan.
When your loved one arrives, our caregivers and staff will do everything possible to make the stay a smooth one and help the new resident adjust.
- What are the benefits of an Adult Day Care Program?
Adult day care allows caregivers to continue working outside the home, receive help with the physical care of a loved one, avoid placing a loved one in institutional care, and have respite from what can be a "24/7" responsibility. The care receiver can also benefit from adult day care.
He or she is able to remain at home with family and have the opportunity to interact socially with peers, share in stimulating activities, receive physical or speech therapy if needed, and receive assistance with the activities of daily living with dignity.
A day at our adult day care center could include: supervised care; small group and individual activities such as reminiscence, sensory stimulation, music, art, and intergenerational activities; nutritious meals; transportation; case management; recreation and exercise; nursing care; education; assistance with activities of daily living; and occupational, speech and physical therapies. These services are customized to each participant’s needs. Some therapies require a doctor's recommendation.

- Is it time for Assisted Living?
When the normal tasks of day-to-day life become difficult, it is time to make a move. At Willow Creek, Assisted Living can be a gradual transition. Services can be added as needed, and we will provide evaluations to see what kind of help might be appropriate.
Assisted Living offers non-medical services like: assistance with activities of daily living, bathing, grooming and dressing, transportation to medical offices and shopping, medication reminders and meal preparation.
A health assessment is conducted prior to move-in. The resident, family members, significant other, and Willow Creek staff develop an individually-tailored service plan together. The service plan is updated periodically to assure that, as the resident’s needs change, the services are adjusted accordingly.
- May I bring my own furniture?
Of course! Our apartments are unfurnished and residents are encouraged to decorate to their own taste.
Each apartment has a private bath and shower, as well as its own kitchenette that includes a small refrigerator, microwave oven, sink, and counter space. Flat linen laundry services are provided. Cable TV is provided in all units as well as the TV Room.
The floor plans have been designed with the needs of the senior adult in mind, with space to move independently.
- Will I have privacy if I want it?
Yes. We encourage a social environment outside of your apartment, provide many activities and opportunities for field trips and entertainment. It is up to the individual to decide which activities to participate in.
In addition, any assisted living services are provided with your privacy and dignity in mind.
- Will I lose my freedom?
No. You can still do all the things you enjoy, come and go as you please, have guests at any hour. The only change is that you will have help with things that are hard for you.
Willow Creek will provide the services you require to make your life safe and convenient. Your privacy is safe-guarded, and care is given discretely and with love to preserve your dignity.

General Questions
- Do I have to participate in the activities?
Activities are available for those who wish to participate and we encourage residents to attend those they enjoy and that meet their needs.
The staff publishes and distributes an activity schedule each month that outlines upcoming events.
There is a wide variety of activities available, including baking, bingo, outings, cards, gardening, crafts, chapel services, exercise classes, discussion groups, movies, and live entertainment. Special classes are arranged on a regular basis. Willow Creek furnishes most of the supplies for these activities.
Shopping trips to nearby stores, library trips, and other outings are available on a regular basis in addition to special event outings.
Willow Creek provides free scheduled transportation for trips and outings, although there may be a charge for certain events that the resident would pay for.
- How can I get more information about your programs?
We encourage you to explore our main website and the websites of our locations.
You can contact us via email or by phone to ask questions or arrange a visit. We love to have interested people come to visit us and experience our communities first hand.
- How will my prescriptions be handled?
The staff monitors medications prescribed by a doctor, and a doctor's order is required for all medications. You may self-medicate if you have orders from your physician.
The staff can order medications for you. We recommend that residents use the community's contracted pharmacy provider because they provide additional services that are generally unavailable from other providers including:
- The pharmacy is on call 24 hours a day, 365 days a year, to handle emergencies and answer questions.
- The pharmacy provides delivery services at no extra charge, including after hours and on weekends.
- The pharmacy provides an entire system for medication distribution and monitoring.
- The pharmacy provides in-service training for medication technicians minimizes risk of potential errors.
- The pharmacy provides concurrent therapeutic interventions that result in better resident care.
- May I invite guests to dine with me?
Yes. All meals are served restaurant style in the main dining room. A nominal charge of $5.00 is applied.
We request that guest meals be ordered in advance and paid for at the receptionist's desk prior to the meal.
- What situations may arise where a resident may not be able to continue living at Willow Creek?
Willow Creek is committed to providing quality care and services so that residents are able to remain in their apartments as long as possible. When the community is no longer able to safely provide for their care and meet their needs, other arrangements will have to be made.
Situations in which we would no longer be capable of providing services include:
- The resident demonstrated behavior that imposes an imminent danger to self or others
- Refusal to pay, or nonpayment of charges
- The facility can't meet the resident's needs with the available support structure. This may include residents who:
- have Alzheiner's Disease or Dementia and demonstrate behaviors that show they are trying to leave the facility, cause harm to others, or cause the inability to provide staff an accurate description of needs
- are permanently bedfast
- require skilled nursing care or other medical supervision on a 24-hour basis
- require gastrostomy care

Naturally, everyone wants to live at home for as long as possible. Home, where the surroundings are familiar and everywhere you look are meaningful possessions and mementos collected over a lifetime. Where there are so many cherished memories. But there comes a time when it just gets harder and harder to continue caring for your loved one at home, and that’s the time to think about Willow Creek Memory Care.
The decision to place a loved one in an assisted care residence is never easy. There are many practical considerations, not to mention many emotional ones. We understand the ambivalence you can feel at a time like this, which is why we’re committed to providing the best dementia care services, while devoting ourselves to the attention and support your family needs in this process.
The Willow Creek staff will help you with the steps necessary to facilitate the move and ensure that your loved one's plan of care is designed to enhance their life.
When you can no longer care for a love one suffering from Alzheimer’s disease or a related dementia, let Willow Creek pick up where you left off. We’re the next best thing to family.
- May I personalize my apartment?
Of course! Our Memory Care apartments are unfurnished, and we encourage residents to decorate with items that make them feel at home and safe, and that will help stimulate their memories.
We provide a choice of private and companion accommodations customized for residents with Dementia/Alzheimer's disease.
The floor plans have been designed with the needs of the senior adult in mind, with space to move independently.
We also offer a variety personal services available 24 hours per day.
- We decided to choose Willow Creek Memory Care for our loved one. What happens next?
Welcome to our family! We will help you every step of the way to make the process as easy as possible.
You will receive a packet with simple paperwork to fill out such as:
- Deposit form and payment agreement
- Admission agreement
- General information about your loved one, including favorite things, diet, and other details that will help our staff relate to them effectively and ensure their stay is enjoyable.
- Emergency Contact information for family, doctors, dentists, etc.
- Consent forms
Your primary care physician will complete and sign the physician's assessment and medical information form, letting us know what medications are required, and providing a prescription order. We will continue to work with the doctor through the resident's stay.

Once all the paperwork is completed and prior to move-in, we will schedule a meeting with the Health Care Coordinator and Executive Director to discuss special needs and design a personalized program for your loved one.
We will give you a list of things to bring on moving day, and there will be more papers to sign.
After the move, our caregivers and staff will do everything possible to make the transition a smooth one and help the new resident adjust.

- What is Respite Care and why should we look into it?
Respite Care provides time off for Caregivers who are burnt out, need rest or healthcare for themselves, or can't provide care for an extended period of time due to other commitments and responsibilities.
The person who requires care can come to stay at a Willow Creek Community for a few days - up to a month or even more. They will receive all the Assisted Living or Memory Care they require, 24 hours per day. They will be eligible to participate in any activities at their location and have the opportunity to socialize. The Caregiver has peace of mind while they take care of other things and refresh themselves.
